Annual Open Enrollment for Retirees and COBRA Participants
2013-2014 Monthly Rates
Retiree/COBRA Benefits are for retirees and former Independent School District 15 employees who were contracted at or above 1020 hours per year and enrolled in benefits at the end of their employment with ISD 15.
All change forms are due to Wendy Carlberg, Benefits, Human Resources by 4:00 p.m. on Friday, June 14, 2013. If no changes are made, no further action is necessary. If change forms are not received by the deadline, benefits will remain the same.

The NeoPath Health Clinic is available for former employees and retirees who are under age 65 and enrolled in a medical plan through Independent School District 15.

Listed below are 2012-13 rates. 2013-14 rates to be determined. Retirees and COBRA participants will be notified of a rate change by mail.

Dependent Life $5,000 Policy: $1.25

Billing & Administrative Fees
Credit card payments will be accepted with an additional 3% service fee. ISD 15 is not able to receive electronic payments.
Administrative fees are as follows:
- Annual payment: no charge
- Quarterly payments: 1% fee
- Monthly payments: 2% fee
Billing administrative fees and premium billing cycles will remain the same as 2012-2013. If you wish to make changes to your premium billing cycle, please email Lori Davis or call 763-753-7051.
